Those included are: GPs, medical practitioners, CAMHS workers, social workers, members of the Children’s’ and Families’ Practice team, members of the Strengthening Families’ team, youth workers, youth offending workers, parish workers, School staff and pastoral leaders for example.
Parents/ carers who would like to see their child referred need to consult one of the professionals above who is working with their child.
Referring a child – The Process
- If you are a new referrer to Ride High, you will need to complete and submit the Referrer’s Service Level Agreement at the same time as the child / young person referral form. It is important to gain consent from the parents / carers to make the referral and this confirmation needs to be shown on the referral form where indicated.
- If it is possible to have the parent/carer consent form completed and signed, this can be returned along with the referral form. If you expect that the child’s living arrangements may change, the parent/carer consent form can be completed at the time at which a place becomes available for the young person after a time on the waiting list. Parents/ carers can be asked to complete the form when the Ride High staff visit the home to meet the child/ young person and family before starting.
- It is vitally important that the referral forms are competed with as much information as possible to enable us to appropriately prioritise the child and place them on the most appropriate waiting list. If there are sensitive details that you do not wish to disclose on the form, please contact the Children’s Manager to discuss in confidence. Please note if forms do not contain enough information then we may return them to you for further details.
- Contact details are very important. We acknowledge referrals to all referrers within 5 – 10 days so if you have not had confirmation of receipt please contact us. Email is the quickest referral method.
